IWU ATHLETIC PHYSICALS:

All first-time freshman and transfer students are required to have a medical physical completed prior to their arrival on campus. An athlete WILL NOT be allowed to participated without a physical. The physical must be performed by a MD or DO, physician assistant (PA), or a nurse practitioner (NP). The IWU Pre-participation Physical form MUST be used for an athlete to be eligible to participate.

This physical should be mailed to the Head Athletic Trainer at IWU. For fall sport athletes, this Physical is due to IWU by August 9. Please note this physical form is different than the one the IWU Health Center requires.

IWU HEALTH CENTER PHYSICALS:

The IWU Health Center requires that all new incoming students have a History & Physical form on file at the Health Center. Please note this physical form is different than the one the Athletic Training Department requires. Your physician can complete each form at the same time, but each form must be filled out and submitted.

The Health Center requests that you send their form directly to the Health Center by August 15. Please DO NOT send the IWU Athletic Physical form to the Health Center. This is particularly a problem for fall sport athletes, as practices begin before the Health Center opens. Therefore, the Athletic Training staff will not have access to those and will not be able to allow the athlete to participate in practices.

MEDICAL INSURANCE FORM:

This form is filled out by each IWU student-athlete and their parents. In case of an injury, it provides the Athletic Training Department with the necessary information regarding the athlete's primary medical insurance.

SECONDARY INSURANCE FOR ATHLETIC INJURIES:

Indiana Wesleyan University provides a secondary athletic accident insurance for varsity intercollegiate athletes. This insurance is meant to supplement the athlete's primary medical insurance, and is limited to injuries sustained while participating in varsity athletic practices or competitions.
