All first-time freshman and transfer students are required to have a medical physical completed prior to their arrivial on campus.  The physical must be performed by an MD, physician assistant (PA), or nurse practitioner (NP) and MUST be completed on this standard form for an athlete to be eligible to participate at Indiana Wesleyan University.
