All first-time freshman and transfer students are required to have a medical physical completed prior to their arrival on campus.  An athlete will not be allowed to participate without a physical.  The physical must be performed by an MD or DO, physician assistant (PA), or nurse practitioner (NP), and MUST be completed on this standard form for an athlete to be eligible to participate at Indiana Wesleyan University.  Please note that this physical form is different from the form that the IWU Health Center requires.

MEDICAL INSURANCE FORM:
This form is filled out each year by IWU student-athletes.  It provides us the necessary information regarding the athlete's primary medical insurance.

Indiana Wesleyan University provides a secondary athletic accident insurance for varsity intercollegiate athletes.  This insurance is meant to supplement the athlete's primary medical insurance.  This handout provides information which outlines this secondary insurance policy.
